What is Support Coordination?

Support Coordination increases your capacity to make the best use of the supports in your NDIS Plan. Support Coordination falls under the capacity building section of your funding and good NDIS Support Coordination services will:

  • Spend time helping you understand and use your NDIS Plan to pursue your NDIS Goals
  • Provide Support Coordination training to build your skills
  • Connect you with NDIS providers, community, mainstream, and other government services
  • Help you build your Support Coordination Toolkit
  • Build your confidence and skills to use and coordinate your supports

What does a Support Coordinator Do?

What a support coordinator does is dependent on YOUR individual circumstances and care needs! Some support coordinators have specialist skills in certain areas that will help with specific NDIS Goals. For example, Care CC specialises in Supported Independent Living, School Leaver Employment Supports and Complex Care needs. Despite this, many duties of a support coordinator are consistent, including.

  • Build Capacity to understand your NDIS Plan
  • Coordinate the support you need

What are the three levels of Support Coordination?

If your plan does not include Support Coordination services and you are struggling without it, please contact us directly today. If you have received Support Coordination in your NDIS budget, it will fall under one of the three levels of Support Coordination Services.

  • Support Connection: Under this level of coordination, Care CC is tasked with building your capacity to connect with informal, community and funded supports to smash your NDIS Goals.
  • Support Coordination: Care CC will build your skills and help you to understand how to use your plan. We will increase your capacity to manage service delivery, live more independently, increase community participation, and maintain relationships through a mix of supports.
  • Specialist Support Coordination: Not everyone is qualified to provide this higher level of support. If your situation is complex and you need extra assistance, CareCC will use their higher-level qualifications and lived experience to manage the challenges of a support environment and maintain consistent service delivery

What makes a good Support Coordinator?

Once you receive an NDIS Support Coordination budget, you may begin asking yourself:

To summarise briefly, a good Support Coordinator will be innovative, reliable, educated, and ideally come with lived experience. Choosing your NDIS Support Coordinator should feel natural, you should like the person who is going to look after your care, finding them approachable and easy to talk to. When looking for a Support Coordinator, you should ask them whether they possess the following skills:

  • Lived experience of living with a disability or caring for persons with a disability
  • Complex communication skills
  • Teamwork
  • Customer Service
  • Proven history of meeting deadlines
  • Report writing skills
  • At least Cert. Three Qualifications in social work, psychology, medicine, allied health, and Community Services.

What can a Support Coordinator claim for?

A Support Coordinator can charge for services according to the NDIS price guide. The Support Coordinator will also ensure any proposed mix of supports are charged in accordance with the NDIS pricing arrangements and price limits. Below are a few things Support Coordinators can claim for, although not all Support Coordinators will charge for all these services.

  • Support Coordinators can claim travel in relation to a support item if the NDIS Pricing Arrangements and Price Limits indicate that they can.
  • Support Coordination Level 1: Support Connection can be charged at between $70.87 per hour and $106.31 dollars per hour using the NDIS Line-Item Number: 07_001_0106_8_3
  • Support Coordination Level 2: Coordination of Supports can be charged at between $100.14 and $150.21 dollars per hour using the NDIS Line-Item Number 07_002_0106_8_3
  • Support Coordination Level 3: Specialist Support Coordination can be charged at between $190.54 and $285.80 dollars per hour using the NDIS Line-Item Number 07_004_0132_8_3

What is the difference between a Support Coordinator and a Plan Manager?

An NDIS plan manager is in charge of ensuring your care providers are paid in a timely manner according to the NDIS price guide.

A Support Coordinator will ensure the supports you receive are of a high quality, are acting in your best interests, are reliable and are acting in accordance with all relevant NDIS legislation.
