SERVICES SERVICES

We Provide

Support Coordination, Specialist Support Coordination, Psychosocial Recovery Coaching, Innovative Community Access and School Leavers Education Support. We support all ages, all disabilities and including those with complex behaviours within the community, acute and forensic settings.

We Are Qualified

Our Psychosocial Recovery Coaches and Support Coordinators are all qualified with the relevant qualifications, lived and or learnt experiences, building quality relationships, trust, empathy and partnerships with people.

Our professional team at Care Coordination Consultancy consists of Skye Hartung (CEO), BaPsychSc MMHPrac, who has over fifteen years of experience working with people with mental health, disabilities and  alcohol and other drugs to experience recovery and rehabilitation within the community, acute, and forensic settings.
